Membership Terms and Condition

1. Introduction

 

These Terms and Conditions govern your subscription with House of Health. By subscribing, you agree to a 12-month minimum contract for our health services, which includes [link to the subscription tier breakdown document]. These terms set out the rights and obligations of both parties.

 

 

2. Subscription Plans

 

House of Health offers an all-inclusive health subscription that provides access to:

 

– General Practitioner (GP) consultations (subject to the Fair Use Policy)

– Blood tests for common health markers

– Annual comprehensive health check (1/person/year)

 

 

Subscription Pricing:

 

– Single Membership: £70/month

– Couple Membership: £120/month

– Family Membership: £150/month (2 adults + up to 2 children under 12)

 

 

3. Membership Period and Cancellation – Initial Term:

 

Your membership begins on the date of registration and is valid for a minimum of 12 months.

 

– Automatic Renewal: After the initial 12 months, your membership will continue on a rolling monthly basis unless cancelled in writing at least 30 days prior to the renewal date.

– Cancellation: You may cancel your subscription after the first 12 months. Early cancellation within the first 12 months will result in a penalty equivalent to three months’ subscription fees.

 

 

4. Cooling-Off Period

 

You have the right to cancel your membership within 14 days from the start date for a full refund. If any services are used during this period, their cost will be deducted from the refund.

 

 

5. Appointments and Access to Services

 

– GP Consultations: Access to GP services is subject to our Fair Use Policy. Members may not exceed 6 GP consultations per year as part of the membership. Additional appointments can be booked at a discounted member rate.

– Health Checks and Blood Tests: Members are entitled to an annual comprehensive health check, including a range of diagnostic blood tests (full details to be confirmed after an assessment with our specialist nurse).

– Pre-Health Questionnaire: Members are required to complete a pre-health questionnaire within 14 days of registration. Failure to complete this form on time may result in membership cancellation at the discretion of House of Health administration.

 

 

6. Fair Use Policy

 

To ensure the availability of services for all members, a Fair Use Policy is in place:

 

– GP consultations are limited to 6 appointments per year per member.

– Abuse of the policy, such as excessive or unjustified appointment bookings, may lead to a review of membership eligibility.

 

 

7. Payment Terms

 

– Monthly Payment: Subscription fees are payable monthly, in advance, via direct debit or credit card.

– Annual Payment: Members may also choose to pay the annual fee upfront.

– Non-Payment: If payment is not received within 5 working days of the due date, your membership will be suspended, and access to services will be restricted until full payment is made. By providing your direct debit details, you agree to these Terms and Conditions, including the Fair Use Policy and payment requirements.

 

 

8. Prescription Services

 

Medications prescribed by our GPs can be delivered via our pharmacy partners or collected from your local pharmacy using a prescription code. Prescription costs are not included in the membership fee and will incur an additional charge.

 

 

9. Termination of Services

 

House of Health reserves the right to terminate your membership if:

 

– There is repeated failure to make payments.

– You breach any terms outlined in this agreement.

– The pre-health questionnaire is not completed within 14 days of registration.

 

 

10. Data Protection and Privacy

 

Your personal and health data are protected under the General Data Protection Regulation (GDPR). We will only use your data to deliver the agreed services and will not share it with third parties without your consent.

 

11. Liability

 

House of Health is not liable for adverse health outcomes resulting from missed appointments or failure to follow medical advice.
